If you have a referral from your doctor for a sleep test with Pacific Sleep, you can send it to us by clicking on the link below, and completing our Patient Registration Form. Medicare requires some screening questions to be done, and completing this form will enable us to make an appointment for you more quickly.

Talk to your GP or other doctor about a sleep referral for a test with Pacific Sleep. You can download and print this form for your doctor to complete, as you will need it for your first appointment with Pacific Sleep.
